The hospital you were born in.
city hall in the city you were born. Costs a few bones.
with the proper id to recieve a new one. usually ss card and drivers id.
You have to send a letter to the town hall that you were born in, and it has to be noterized.
I know that when I sent for mine, a copy cost $10
I had to get a new one last year, and I just went to the town hall where I was born. $10 and one week later I had it.
I was born in NT. Went to city hall, paid $10.00 and picked it up. Took 5 minutes.

born in buffalo? go to city hall and buy a copy. I did this, but it was ~11 yrs ago. IIRC it was $10.
