Not my motor, but a fellow luder over on PreludePower had a valve head fall off. This was the end result:

Yea, that engine is done. He lost 3 valve heads in that cylinder. Sucked the resulting metal bits up into the intake manifold and shot them into the other 3 cylinders destroying them as well (though to a lesser extent).
