I just did this Saturday for my trailer plate that was destroyed by years of running into stuff and wind;
I first went to the DMV and said I need new plates. “you need to go to the police and fill out MV-78B”
I then handed over a piece of plate that was the size of a quarter and a piece that was 3" long. “you need to go to the police and fill out MV-78B”
So… I went to the police and filled out MV-78B.
Back to the DMV and done.
An answer. So it is a official form. Fuck the city of Buffalo and making me waste yet another day downtown looking for this damn form.

I’ve been w/ DMV for 12 years and i dont know what that form is. i tought a 78[B] was the green receipt you get when you turn in plates. i’m gonna have to look it up tomorrow at work
I asked my friend that works for gieco and she said you will get credited from the date the plates were destroyed. good luck with that though.

Why not transfer it to the rabbit?
It would be easier to just keep paying the insurance for as long as you wanted to keep the plates.

Is it actually notarized by a notary, or just on official letterhead that is “pretty much” notarized with some worker’s signature? They are not the same in DMV eyes.
Good luck with the other suggestions. I would have reported them lost or stolen the minute you decided to let the city keep the car.
